Dear Patient,

patient surveyWe would like to know how we can improve our service to you and how you perceive our surgery and staff. 

To help us with this, we have set up a succesfull patient participation group (PPG)  the puropse of the PPG is to ensure that patients are involved in decisions about the range and quality of services provided by Lockfield Surgery. This includes being involved in decisions that lead to changes to the services that the practice provides. This will involve a group of patients meeting (usually on a Saturday morning) every 3 months, meetings last approximately 1 - 1 1/2  hours.  All patients are welcome to join our PPG by either enquiring at reception, telephone or by e-mail at lockfieldsurgery@walsall.nhs.uk  We aim to gather patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
